CARE NAVIGATION

Our receptionists are here to help you get to the right service. Please don't be offended if they ask what the problem is when you call to make an appointment. The receptionist might suggest other professionals that could help you such as: Opticians, Pharmacist, Nurse, Family Services or Support Groups. Thank you for your co-operation with this.

New Patient Registrationregister_now

If you live within our practice area and would like to register please ask for a registration form and information pack at reception. Forms are also available to download from the above "Registering Online" tab.

We also require one form of photo identification and proof of address. You will be asked to complete a simple health questionnaire so that the doctors and nurses have some information about you immediately.  An appointment will be made for a new patient health check at the time of registering with the practice.

Health checks provide an assessment of your height, weight, blood pressure and discussion/advice about lifestyle such as diet, smoking, alcohol consumption and levels of physical activity. You will be required to bring a urine sample with you

If you are on repeat prescriptions for on-going medicines you will be asked to make an appointment with the doctor. We suggest that you try to obtain a month’s supply of your medicines from your previous practice to give us time to process your new repeat prescriptions.

If you have any questions please do not hesitate to contact any member of the team.
